Thank you for joining our Medicinal Herb CSA program. A big part of making this offering as supportive as possible is due to the input from members. Your feedback allows us to assess what is working well, what might be challenging and helps us ensure that the CSA program is truly responsive to the needs of our community.

Your responses will primarily be used for internal use and aggregated data may be used for grant reports that are shared with funders (keeping individual member information anonymous). We may pull key quotes and photos to share for promotional purposes if you consent to this.
